This is the title that will appear at the top of your browser window. If your browser is set to print the webpage title at the top of each printed page this title will appear there.
Most web browsers are set this way by default, so unless someone has changed the settings it should appear there.
Page Title:
Same intent as above Browser Title except that this one prints on the page itself. If your browser is for some reason NOT setup to print webpage titles, you can leave the above line blank and put that text here instead. In a pinch this might also be used for a second line of text when your browser IS setup to print webpage titles. Just experiment with these two lines to see what works for you.
Subject:
This is the title that will appear at the top of the worsheet. If you generate a worksheet with too many problems to fit on one sheet the title will only appear on the first page of the set of worksheets. It would be best to try a few sample worksheets to see how this really works.

This is the date that the assignment will be due.
Name:
Instructor's name: Can be formal (Ms. Jones) or informal (Ed Michel). It's your choice.

Some browsers may misbehave and not allow you to pick the numbers of columns and problems you want. Most problems are caused when the browser gets confused about how to handle a large number of columns. Your browser may decide to 'Word Wrap' the individual problems or change the margins to fit all of the columns. In any case, if you have a problem with the layout, try lowering the number of columns. If you desire a large number of problems and the last row on the first page gets split, try increasing the number of columns.
